Return to Play: Player and Parent Questionnaire
As we begin to return to play in the wake of COVID-19, we know that practices and games won’t look the same for a while. That’s why we want to hear from you. We hope you’ll take a few minutes to answer this short survey so that we can run our programs in a way that makes you and your family feel safe and comfortable.
How comfortable will you be sending your child to practice/games when the stay-at-home order is lifted?
A. Very comfortable. We can’t wait.
B. It depends on what the club is doing to protect players.
C. I will not send my child until there is a vaccine.

What factors are more important to you when deciding whether or not to send your child to practice/games? (Select top two)
